Coming home from hospital is a big step, especially after a fall, an operation or a long illness. A little planning makes the first few days much safer and less stressful for everyone.
1. Talk to the ward team early
Ask the ward or discharge team what support is planned, when discharge is likely, and whether any equipment or short-term help will be provided. Write down names and phone numbers.
2. Think about the first week
- Who will be there on the first day home?
- Can they get in and out of bed, wash, dress and use the toilet safely?
- Is there food in the house and are medicines ready?
- Is anything needed, such as a key safe, grab rails or a commode?
3. Arrange care before the discharge date
If extra help is needed, contact a care provider as soon as possible. A home care assessment can often be done before discharge, so carers are ready from day one.
4. Keep it flexible
Needs often change in the first few weeks. Short-term care can be increased, reduced or stopped as someone recovers, and respite can give family carers a break.
5. Check what funding is available
Some people get short-term support after hospital, and some may be eligible for council or NHS funding. Others choose to arrange care privately. Ask the discharge team, and read our guide to paying for home care.
